What's Thai Official Form Mean Exactly ?

I think I have to see this document in this forum, it's something it allow to leave the child out of Thailand,

but I don't know any more what's mean. It's seems impossible to take the text from PDF to translate into Google.

Can you explain what's mean exactly this documents ?

and which admnistration need to apply ? usually in living in Bangkok.. or province.. Thanks : )

With this paper you give permission to someone to travel with your child(ren) to another country.

You also say that this person is allowed to apply for a passport for your child(ren).

I am just guessing, but I think the person travelling with your children would need this paper in the passport office and in the the airport when leaving the country with your children (you better give him/her also a signed copy of your passport).

Also, I think both father and mother need to agree and thus both have to fill in the such a paper and both papers have to be signed by the father and the mother.
