I do not think that with the new security features of the passports these days the option of adding pages is there anymore and you will find you'l have to apply for a new passport

I cannot answer the original question, but I had pages added to my US passport in BKK just a few months ago. My wife actually went by and did it as part of another errand to that side of town. She took the "add pages" form I had filled out and also a limited power-of-attorney form as I recall. So who knows, maybe it is possible? I'd call or email as Lopburi suggested.
