Does anyone know if it is possible to get a double entry tourist visa from Hong Kong for a USA passport holder.

They only do single entry tourist visas. And you may need to show a ticket out of Thailand to get it.

Do you know if that is the same for getting a 2 entry visa from Tokyo?  I need a two entry visa as I am doing extensive dental proceedures in Bangkok and I do not have a return ticket booked until the dentral work is finished.  I need about 5 months to complete the work.

You need to be a legal resident to get a visa in Tokyo.

Better to fly in with a full refundable ticket, cheap ticket or reservation, then get tourist visa in Savannakhet.
