I'm going to apply in USA. My school hasn't given me any forms and can't seem to figure out which one I need. Can I just use the same form? That looks pretty similar to the one they use in Laos...

Most embassies and consulates have their own forms. The one you posted a link to is a generic one from the MFA.

Look on the website for the location for where you are going to apply.

Does anyone happen to recall what color background the photos are supposed to have for visa applications?

Most embassies and consulates are not that picky about the photos. The default background is a white or off white background.
