She could first get a 30 day extension of her 30 day visa exempt entry at an immigration office for a fee of 1900 baht.

Then if she needs more time perhaps do a border hop for another 30 day entry.

FYI children under the age of 15 do not pay overstay fines but do get overstay stamps (other than perhaps at Phuket).

If we followed the process you suggest, are there any documents needed from her parents?

You should have a consent form from her parents that could cover many things including medical care.

For the 30 day extension it might be asked for when she applies for it.

Is there a specific form for this or will a comprehensive letter from parents be good enough?

There is no standard form for it. Normally it is done as a notarized power of attorney.
