Soon i need to make an extension of tourist visa. I want to know what i need ? I have a TM7 with pictures, i need something else ?

I'll make an extension at the immigration office in phuket but I live in phang-nga, is it a problem? Do i need a adresse in phuket ?

If you are not staying in a hotel, they may insist on a TM30 (and fine you if one has not been submitted in advance).

In addition to the form you will need

1. A photograph

2. Copy of you passport data page

3. Copies of the visa and entry stamp

4. Copy of the departure card

5 Evidence of a local Phuket address.

There is an immigration office in Phang-nga. See: Address and Google Map location of immigration offices

If you apply in Phuket you will need an address there to put on the application form and possibly proof of it.

You will need copies of your passport photo page, visa, entry/permitted to stay stamp and TM6 departure card.

And last question, how i can proof my adresse ? I rent a room in Phang-Nga, the owner never ask me a name or something.

Not sure if Phang-Nga will ask for proof of address or not. A receipt for your room with the address shown on it should be enough at most offices.
