My normal practice for the past year or so is to get a 30 day exempt at Nong Khai and then a 60 day extension based on marriage from Khon Kean Imm Office. I do this simply as an excuse for visiting friends in Udon and KK, otherwise I'd never get out.

My current exempt runs out today but due to ill health I'm unable to go (some 240km round trip)

I have resigned myself to the fact that I'm going to have to overstay (hopefully only until Monday)

My question is if it is possible to settle overstay at Khon Kaen office and get a 60 extension at same time, or do I need to go out again.

If I have to go out this will be 2 u-turns in a row. Is this still possible or is a visa from Vientaine needed.

Cheers for any help

Immigration can let you pay the fine of 500 baht for overstay and then do an extension. But it can depend upon the office you go to. One day of overstay would never be a problem but you will have 4 days if you go on Monday.
