ubonjoe
A copy of your wife's house book will be needed as proof of residence if it is for where you are living. If you had your stamps transferred at immigration you will not need your old passport. You will need a copy of the stamp they did that has info about your old passport and the visa you used for entry. The normally will want a copy of every page of your passport that has anything to do with Thailand on it and the photo page. Copy of TM6 departure card.
