Extending First Entry Of Multiple-entry Tourist Visa

I have a 4 entry tourist visa valid for 6 months (from Thai embassy in London - I'm British) and my first entry is coming to an end. I have a prolapsed disc in my back so Lopburi helpfully suggested I extend my first entry rather than do a visa run which might be a bit rough on my back.

I haven't extended a visa before in Thailand and would be grateful for advice with the following:

What documents/reasons do you need for a visa extension, how long do they take to get and can I be confident that it will be granted for 30 days? Can I do it in one day on Monday at the sathorn immigration office (I guess it will be closed on the public holiday on tuesday)?

Once I'm given this how does it effect the rest of my multiple-entry visa? eg have I used up my second entry or do I still have 3 entries left?

Fill out the tourist visa A/F and state reason for extention as 'tourism', take it with a photo and photocopies of your passport personal info page, your entry visa and entry card and pay 1900 baht. Take it in in the morning, and collect it in the afternoon.

Download A/F .pdf (above pinned documents).

No Problem.

This will be a 30 day extension.

Then you have three more valid entry into Thailand left. Do your last entry a day before your quad visa expires.
