Tourist visa 30 day extension form

Hi,

 

Regarding tourist visa 30 day extension form, specifically the "form for house-master" portion. A slip attached to the form suggested I needed copy of ONE of: rent contract, house book or owner id card, however I felt the girl at immigration suggested I needed all three.. was not quite clear. What do I actually need?

 

Second, as the owner is out-of-country right now, he can only email me back the completed form. I assume this will be OK?

That want the rental agreement with copies of the house book and ID card attached.

The may accept a emailed copy of the form but could reject since it will not have a original signature on it.
