There is a re-entry permit office is just a after departure immigration at DMK.

Be sure you take photo for the application form in case they cannot do it.

Some time ago they used to fill out the form on their computer and take your photo right on the spot as a compulsory 200 baht service. Last week this was not the case, you need to bring your own photos and fill out the form manually.
Tell the immigration officer (the one at the passport check) that you need a re-entry so he gives you back your stamped departure card to bring to the re-entry facility.

I'm flying internally to Hat Yai then crossing into Malaysia by land,  all on the same day. What are my options for getting a re- entry permit?

Getting one at a immigration office before starting your trip.

Not sure about it but have read that there is a re-entry permit desk at the Sadao crossing.
