1 year EXTENSION based on retirement----Statements?

Do you need 3 months or just the latest month proving your monthly income?

 

 

Doe the USA embassy charge by the page it notarizes or is there a flat fee, like per visit?

The US Embassy requires you sign a paper stating your monthly salary - there is no requirement for statements as your signature makes it a legal document.  The charge is for that document.  You can fill out and download prior to visit but do not sign.

Have you checked there website?  Remember you also have to make reservation for service using website.
