    i did mine a few weeks ago and like you i didn't provide any additional forms, but at the end the IO handed me a couple of forms to sign, said nothing, no time to read. I reckon those forms were the ones mentioned in the op

    AIS has update your ID all over their website, so i email them to check, lady calls me back to say my ID is ok but you need to go to branch to sign some forms, new terms or something. So i go to AIS and they said everything ok, no need to change or sign anything. Typical

    because we know Jomtien could just use it as an excuse to send you to the back of the line, hence why most sensible people prepare all docs in advance
