Apparently he flipped out when he heard the lineup of A-list celebs attending Biden’s inauguration — all people that turned him down for his own one. Actually that’s both the case. The diseases are specified in the law, and only doctors and hospitals can issue such certificates. 
It’s the diseases that don’t seem to make sense. When you have 3rd stage syphilis, you don’t need a test for that—you’ll be screaming it all over. I think the same is the case for elephantiasis. Alcohol addiction may not be easy to diagnose. Forgot the other two.

The current medical certificates aren’t that expensive. It’s a blood test and a doctor looking at you. Most companies reimburse you for it (or pay directly to the hospital).

I’m on a B visa with work permit. AFAIR I needed it for my last extension (as well as for the initial application process). Maybe that’s the difference.

AFAIR you need to go through the medical certificate thing for every extension, and the doctors and hospitals are not handing out carte blanche certificates anymore as they used to many years ago. I believe I had to do it for my last extension.
