The passport number is not that big of a problem. If you name and date of birth is correct that will be enough.
Some vaccine certificates do not have a passport number on them. Most issued in other countries do not have a passport number on them.

Those denied a visa exempt entry only left the country for a few days and entered visa exempt to stay here longer.
You will be away for several months when you return and apparently did not make short trip to a nearby country just to enter with a new entry.
There have ben reports of people being denied a visa exempt entry after being here for a long time and only leaving for a short period of time after being here on the amnesty and 60 day covid 19 extension.
That was the things done at Muang Thai Thai that are now being done at Chaeng Wattana.

Since you were on a extensions of stay for a long time you will not have a problem entering visa exempt.
You may need to show a ticket out of the country within 30 days. But since your planned stay is only for 3 weeks I assume will have a return ticket.
You could be asked to show the equivalent of 20k baht in cash when entering the country.
I am not sure what the insurance requirements will be July when you enter the country. At this time you would need covid 19 insurance coverage amounting to $20,000 valid for 30 days.
Just got approval of my 2nd 90 day report on the new site after submitting it this morning.
Easiest online report ever. Filled in my passport number, selected my nationality and clicked the magnifying glass to complete the application and submitted it.
If you are in Bangkok it is done at Chaeng Wattana immigration. It is building B in the government complex.
Insurance is required.
You should be doing 90 day reports. Your extension will be considered as doing a 90 day report if it is your first one and the next one will be due 90 days from the day you apply for the extension.
There is no 18 months rule for anything you do at immigration. That only exists at Thai embassies and consulates for multiple entry visas.
The only rule immigration has it your passport must be valid the length of the extension of stay you are applying for and they will still do the extension but it will only be issued to the day your passport expires.

It would be great but all we can do is wait for October to see what they do. Even another 5.9% increase would excellent.
I don't think it has changed and all that was needed was to complete a form to confirm you are not a high risk of being infected.
If needed they should accept proof on Mor Prom that you are vaccinated. I am aware of it being accepted elsewhere.
Do you have a source for that info? I suspect it an opinion based upon the current inflation rate in the states.
The COLA increase will not be officially announced until October.
