I did my retirement extension Tuesday, Apr 29th, and was handed the STM 2 Form to fill out when I was having my documents checked. Took about 1 minute to fill out while I stood off to the side. I downloaded the form from the link posted by upnotover and converted it to PDF. I was going to attach it but it is not allowed to attach PDF files.

If you're talking about the TTB Branch on 2nd Rd by Soi Honey I'm surprised that it took a few days for your annual statement for immigration. I've been using that branch since the merger and was at that branch since it was Siam City Bank and got taken over by Thanachart. I have always received my bank letter on the spot on a walk-in basis. Takes about 30 minutes but have never had to come back a different day. I'll be going there again this coming Tuesday (4/29) to get my annual bank letter and then go to Jomtien to do my retirement extension. I always got to the bank right after they open and then directly to Jomtien. Never had any real problems.

Exactly. The folks that are making a big deal about not always having a physical or "wet" signature are just confusing the issue.
