When applying for new work permit, what to do if told by work permit office that previous company, now out of business, did not cancel previous work permit from a few years back.

If it's from a few years back it will have expired already, so not sure why it's an issue. But if they need it formally cancelled you can do that at the labour office yourself using the work permit cancellation form. Either an employee or the employer can cancel a work permit.

Paying a 500 baht fine for the old work permit not being canceled if it is already expired is what is normally done.

Thanks for the helpful advice to all...does somebody have a copy of that form that can be filled out?  Is the fine still 500 baht?

The DOE website no longer has the cancellation form on it. I have not found another source for it.

As far as I know it is still a 500 baht fine.

This is the only copy I have, but I'm not sure if it's current.
