I have asked this question before and know the answer I get the 90 on top if I fly out and back before my spousal Visa expires. My question is can I extend after the extra 90 days expires ? Probably not I would think

You can extend the last 90 day entry from a multiple entry non-o visa if you want to. You are extending your permit to stay not your visa. Once you are in the country your visas validity means nothing since it only allows you to enter the country.

 Thanks so I can extend in country before the extra 90 days expires

Yes you can. You can apply for a one year extension of stay based upon marriage or 60 days to visit your wife.

I presume by Spouse Visa you mean a Non Imm O Multi entry Visa.

 

You can extend each 90 days permission to stay by 60 days at local Immigration. 1,900 baht.

If you use this Visa to it's potential, you can stay for a total of almost 17 months and only make 3 border crossings.
