I don't disagree with improving one's quality of life. However, perhaps you could explain the distinction between addiction and dependency.

Sure. Generally addiction involves abuse. Eg the term "drug addict"

Dependancy is a term for those truly dependant for pain relief and not getting high for the buzz

Of course there is some blurring of the lines but you get the picture

Addiction (compulsive drug use despite harmful consequences) is characterized by an inability to stop using a drug, failure to meet work, social, or family obligations,

and, sometimes (depending on the drug), tolerance and withdrawal. The latter reflect physical dependence in which the body adapts to the drug, requiring more of it to achieve a certain effect (tolerance) and eliciting drug-specific physical or mental symptoms if drug use is abruptly ceased (withdrawal).

Physical dependence can happen with the chronic use of many drugs—including many prescription drugs, even if taken as instructed. Thus, physical dependence in and of itself does not constitute addiction, but it often accompanies addiction.

I go to the Government Hospital every 4 months for follow up appointments following ACDF surgery.
At my last appointment, 2 months ago, I requested 120 Tramadol, to last me until my next appointment at the end of November. The specialist obliged, prescribed 120 tablets, and my bill was 50 baht consultation fee, and 120 baht for the Tramadol, total 170 baht.

1 baht per tablet. How do they even make a profit on that

Are you serious? My close friend owns a few pharmacies. He told me he pays as little as 10 Satang per tablet for some medications produced in factories in Thailand; provided he buys about 5,000 at a time. At 1 Baht each, that is 90% profit on some meds! I don't think my friend sells anything at 1 Baht each, more like 2-3 Baht (or 180-270% profit).

Digger, I can't believe you are taking that much Tramadol, I was addicted but I drew the line at 250 mg per day.  Do you have any health issues from taking that amount for so long?  Plus, 400 mg is the recommended maximum daily dose, do you get the urge to take more than that?

I have been taking Heaps of anti inflammatories and painkillers since 1985 .I even tried Q10 /shark cartilage/chondroitin sulphate and glucosamine and a few others that I can't remember for 2 years straight, the only thing I found that it cost me an fortune Didn't do Zip.Also I was on anti inflammatories and panadeine forte for years.Nah no Health problems .I even tried to take less Slowly over a couple weeks but that only give me more pain. I don't have the urge of taking more. I am trying to find an different product but they are limited  here.

Be careful with tramadol, if you get into the habit of taking a few more to get a stronger effect, it can cause seizures, which is even more likely in people of advancing age. Tramadol is unique amongst opioid type painkillers as it has the added effect of something akin to an antidepressant.
