Extension of Stay Application Process for Non-Im O-A Retirement

Can someone clarify what documents are needed to satisfy immigration to get an extension of stay of my O-A retirement visa. I am confused...

My situation is as follows. I have had the 800K in the bank for more than 3 months. I have applicable health insurance with Pacific Cross.

What I am confused about is:

Which application form is used and where can I download it?

What documents/copies of bank records are needed?

Is the insurance certificate issued by Pacific Cross via email the only thing needed?

Are there any other documents needed or recommended I take?

What is the application fee?

You use the same TM7 form used for all extensions of stay. Fillable TM7 form.pdf

What is needed from the bank can vary from office to office. Normal is a letter from your bank confirming you account and the balance of the day it written. Copies of updated bank book to prove the 800k baht has been in the bank for at least 2 months on the day you apply. Some office may want a bank statement for the proof as well.

The insurance certificate will be needed. They will also check online to confirm it is valid and the date and time it will go into effect.

Copies of your passport photo page, visa, entry stamp and TM6 departure card.

The fee is 1900 baht.
