My last 90 days report is missing (I did it by mail and never got a confirmation).

I later went abroad for the smog season.

Yesterday, I extended my retirement extension of stay  - they didn't ask for TM47.

Is it safe to assume that a trip abroad makes all TM47 before that date irrelevant?

Or was I just lucky?

It does not make the past disappear.  If an officer checks and sees it was missed, they can still fine you.  That being said, just because you didn't get a confirmation back doesn't mean your mailed submission was not processed.

As mentioned above, most likely it got processed but the reply went astray.

That said it's an interesting question - any cases where people have been fined for missing 90 day reports from previous trips?

Normaly they will not check for missing 90 day reports. They just check the time since your last arrival. But as before said, this is not sure, If an officer checks and sees the missing 90 day report, he can still fine you.

It is never safe to assume anything in life:-) I would just carry on regardless as you did with the extensions and next TM47 until or if they pull you up on it.
