There seems some confusion in some quarters as to when a Thai woman is legally married at an Amphur is it mandatory or optional that she changes her family name to that of her husband?

Sunbelt may be able to answer this one. My wife changed hers in 2005 when we were married.I think it makes things easier if you intend to live in Thailand.

Experiences of TV members in Thailand? Has there been any amendments to the Regulations concerning the above?

The requirement to change was abolished a few years ago. But there is still the requirement to change Miss/Mrs on home register within 60 days and all id will have to be changed after that.

The Amphur was in the local district where she lives in Bangkok, they may not have known about the change then in 2005. Different offices may have different rules, TiT.

So let us say it is now optional, but title must be changed.

Name change is not a thing one would do very often, marriage and divorce (where the whole process is reversed!)

Thanks astral, the best reply seeing that we are in Thailand, and yes there can be complications, I could think of two at least.
