Hard to say what the "best" way to travel Phuket to Vientianne would be. If you want to get there with the least hassle you should be able to find a travel agent in Phuket to arrange a air ticket, Phuket to Bkk and a connecting flight on to Vintianne. If you want to be a bit more adventuresome then fly or take the bus up to Bangkok spend the night then get the train Bangkok to Nongkai. From Nongkai it's easy to get to the Friendship Bridge, cross and get into Vientianne. A decent and inexpensive place to stay in Vientianne try: Mali Namphu Guest House, www.malinamphu.com for details.

You can get the Lao visa at the border crossing. Thai passport holders dont need a visa. I assume you mean a reentry permit to get back in to Thailand. You better get that at the Immigration office in Phuket as you cannot get it at the border.
