Just for fun I screen grabbed a frame from the movie "Chocolate":

With A LOT of help from Thai-language.com I decoded this as "building patients in"

It could be a direction/sign i.e. "building's entrance for patients", but I don't think that's the right 'in'. I'm reminded of the UK term "in-patients" for patients who are admitted for treatment. So could this mean "in-patients building"?

The small print had me misreading the last syllable as bai instead of kai, but on closer examination I've got "medicine - ? - egg" - so I'm thinking your advising me not to ask for fertility treatment? ( or drop-egg -> birth? maternity???)

I'm not familiar with the commercial, I guessing it's a play on words. ยาลดไข่ coming from ยาลดไข้ - medicine that reduces a fever temperature, such as Paracetamol.

The commercial came out a few years ago, but still gets a lot of airtime these days. It's a Farang that goes into the drugstore and asks in a Farang accent ขอยาลดไข่ - 'khai' with a low tone which means 'eggs' or in this sense 'testicles' - so actually asking for 'testicle reducing medication' rather than ลดไข้ - 'khai' falling tone - fever. His reaction when they hand him the medication they thought he was requesting was 'this isn't what I asked for' with a worried look on his face.
