Thanks for the report. Glad things wert well.

But you fogot the bank book copies. That is needed every where.

How about posting the form that Krub sent you. I think it is the standard form that is used for many things that require a medical.

Sorry to hear that Nan is still requiring the medical report. There is no need to make the document availeble for download, as the doctor where you go for a health document will have the form and will fill it out for you.

Actually believe the form would be helpful as hospitals have a number they have to keep track of and may not know the correct one to use if not in the Nan area. I do not believe it is the same as employment (at least is was not some years ago). How forms are addressed can be a very picky issue among some bureaucrats.

Good report, thanks - makes me and I guess others that need to use the Nan office a little easier.

A download of the medical form might be a good idea - did you use a private doctor/ med centre or a Government hosiptal for the check.

I stand corrected. We will see what we can do about this form. If Krub or WeeGB can PM it to one of the moderators then it would be indeed helpful.

Mario 2008, I've PM'd the medical form to you.

Artisi, I used a private Doctor, who also runs the local clinic. He examined me, then filled in the form (In English). He didn't have any of these forms, said he'd never seen one before, so thanks again Krub for the PM.

He couldn't remember the exact date he got his medical licence, so he asked my wife to pick the form up the next day, as his details were in his office. When she collected it, he had also helpfully put his hospitals "stamp" on it. (It was 1966 by the way, no wonder he couldn't remember!).

I'd suggest anyone needing this form also gets some sort of stamp on it, (similar to the ones they use on forms passports etc here). Immigration saw this and didn't seem to read the form too closely, seems to count for a lot!

Cost me 100 baht - well worth the expense, looking forward to another year in the sun, playing with my 4 puppies and enjoying life with my wife and her family, who are all smashing folks.

Attached is a PDF file of the form for the medical certificate, which the Nan Immigration office still requests for the retirement extension, contrary to currently valid extension rules.

Just done Nan Immigration for the first time for retirement extension.

Had all my paperwork sorted out before hand, only foreigner in there, received one years extension and single entry re-entry permit.

Yes they still wanted a medical certificate, mine 30 Baht from our local polyclinic, signed and stamped, nothing in English on there.

Had already done my Bank letter the previous week with our local BKB,which was free and took some 30 mins. WeeGB grip your admin, do'nt get messed about !

I was a little concerned about this 'day before business', however no probs and

Immigration officer studied balance briefly and reminded myself of 3 month rule for next time!.

As I visited UK twice last year I had re-entry permits and stamps and new TM6, Immigration Officer studied my passport for a long time!.

They also inquired about my previous 90 days reports, so showed an old slip. At least we can do that by post with Nan.

As I was leaving the Imm Office, a Farang monk came in, sort of made my day!

A nice 980km round trip, though I did stay the night in Nan. Thanks to this new policy of going to your regional office I must thank Immigration for allowing me to see a lot more of Thailand.

Still at least I learned photocopying is a available in the immigration office - for next time I forget something.

Have to agree the journey was very interesting, some spectacular views along the way.

I am also pleased to hear that the 'normal' form used by local Thai clinics in Thai seems to also be accepted

s this form is really easily given anywhere for Thais for example when they need one for a driving license or others.

In the 'good' old days immigration would only accept the form posted above which if the doctor wants to do it for 'real'

would need x-rays, blood tests, etc.

I wonder what kind of certificates is accepted in Phu Sing, Isaan) as they also still require one

I've been several times in Nan immigration office now, after being pushed away in Chiang Mai. the office is very small, never seen more than one foreigner beside myself. (the immigration officer told me that there are about 20 applications per month!

the office is located opposite the Nan airport, some 100-200m further than the exit to the airport, left side, road 1080 to Pua. there is a sign along the road for the immigration office, drive slow as you may pass it.

the immigration staff was always very friendly, it looked they were happy someone was coming for their work.

the have accepted my color copies of passport etc, my bank letter was about a week old (last year) and no problem what's so ever, I had 2 copies of each document but got one set back.

there is a copy facility but not close, go into town Nan and close to a Mitsubitshi workshop could we do some extra copies, anyway your Thai wife should be able to ask around. Nan has all major banks.

will be there next week myself for extension of retirement visa also. be interested to hear how it all went in your case. though you may have missed this message already

Check location - you will find it has relocated since your last visit.

just I am looking at Google maps and this shows clearly the nan immigration office still opposite the Nan airport. is google right or wrong?

where would the office been relocated too?? any guidelines? street name or so?

ok looks easy to find. seems they have a new office now, with a second floor and copy machine (as I read before);did not have that in the old building.

A report back on your experience would be good - guess most of us are still a little apprehensive on expectations when applying although what is coming back seems positive.

Hello all,

this morning we visited NAN immigration office and I like to summarise our experiences.

we arrived around 09:30 hours at the office.

Location: this is according the last link given and google map, on road 1091 (nan-payao), turn first U-turn and after about 200 M the office is on the left hand, sign in front of the office - drive slowly the last end to prevent an overshoot.

I was used to the old office at the airport, but this office is better and we congratulated the staff immediately, which was very much appreciated by the staff, of whom most of them still recognised us.

I got an extension for visa based on retirement. this costed about 15 minutes.

paper work required:

- filled in application form,

- one pasphoto

- valid pasport

- copy of pasport pages: page with name and issue data, page of visa, departure card, page of last extension, last 90 day notification

- certificate of income from your local embassy

- medical certificate. confirmed to be new and required.

- cost: Bt 1900

Note: -no bank book, nor copies of bank book were asked, though I had all with me.

-no blue house book was asked.

-please note that for an extension based on marriage or support of a thai national other documents are required, for which I do not have experience but sure other have.

after this I requested a re-entry permit, which took about 10minutes

paper work required:

- application form, got it from the immigration office staff and filled it there,

- pasphoto, one only

- pasport

- copy of pasport pages: page with name and issue data, departure card, page of last extension(this was just renewed and office staff made a copy themself),

- cost: Bt 1000 for single, Bt 3800 for multiple

after this we got the 90day notification done, about 8 minutes

paper work required:

- pasport, with last notification slip

- copy of pasport pages: page with name and issue data, page of visa, extension of visa, departure card, last 90 day notification

Note: in NAN it is possible to send all documents by mail and receive the signed bottom slip in about a week, see my note before, the officer gives you a form for the next 90-day notification and a small paper with address details of the Nan Immigration office.

it was all very pleasant, the wife had a lot of jokes and laughs with the office staff, so all in all we spend just over an hour, the actual work was done quicker though;she also bought almost half the inventory of crackers and nuts from the desk; which pleased the lady very much. :)
