OK. I go to immigration in december for extension, want to visit England one time during course of extension.( April probably) Do I ask for re-entry permit at same time as applying for extension?

That would be a good idea as it will save you another trip to Immigration. It will be valid for the length of time as your extension.

You will need a photo for the application. Same size as the extension application.
