I usually go to Soi Suan Plu Thai Immigrations in Bangkok about 5 to 10 times a month helping shepherd people thru their retirement visa extensions, doing 90 day check-ins, re-entry permits, letters of residence, etc. Call it a 'hobby' if you will, but at least I am developing some rapport with the officials I see time and again there.

In April I went there to get my yearly extension based on retirement the Immigration Officer gave me 3 additional forms to fill out. Since then, each and every time I've gone there with other people, at the end of the retirement extension process the Immigrations clerk has given me the same 3 forms.

(Slightly off-topic but FYI; so far my personal best time for getting someone a retirement extension is 9 minutes. Of course that doesn't include time standing outside in line before they open the doors, or waiting for our queue number to be called. I always go at 7:45AM; usually I get one of the first 5 numbers. The 9 minutes is the time elapsed from sitting down in front of an Immigrations officer until we left room 101 with an extension.)

Back on topic;

When I was there last week I finally asked the Immigrations officer for copies of the forms. It is my firm belief that the incredible slow processing time can be expedited greatly by having as much stuff pre-filled out, just like having all your copies and paperwork in the correct order, signed, etc.

These are the 3 forms;

First form; only in english, says you have been made aware of the 800,000 baht rule for 3 months prior to your extension application.

Second form; with the upper half in thai and lower half in english, says you acknowledge the terms and conditions applicable to your visa, agree to follow them, as well as knowingly violating the terms, voids your visa and carries penalties under thai law.

Third form; in thai and english mixed (like the immigration forms) is called, "Statement". It basically lists your name and address, then further states you guarantee you live at such and such address, and are applying for an extension, etc.

I'm just wondering if anyone has filled these or similar forms out at other Immigrations offices in thailand. I checked and they are NOT available at the often times chaotic "form table" where all the immigration forms, pens and glue are kept. Each time these forms were handed out by the Immigrations officer during the extension process.

Sorry for the poor quality but certainly legible scans.

Any info would be appreciated. .. ..

This is highly interesting. Thank you for posting it and uploading the forms, tod-daniels.

Regarding the first two: so they have a law, the Immigration Act, they have rules, the Police Orders, and then they make you sign two slips of papers to acknowledge that you will comply with this law and the regulations. Very original.

The third form with the title “Statement” is unusual. The English translation is bad and thus confusing, but if I understand it correctly this form is not for signature by the applicant for extension of stay but must be signed by a third person, a guarantor, who with his statement under oath confirms that the applicant continues to live a the stated address and that he guarantees “[the applicant] for considering [the applicant] To stay temporary in the Kingdom”. What obligation, exactly, does this guarantor enter into with his signature on this form, I wonder? Perhaps we need somebody who reads Thai to look at the Thai text.

Tod-daniels, does the guarantor, in the signature section of the form called “Stater”, have to present himself at the immigration office and sign the form in the presence of the immigration official?

It does appear to be for a third party to sign, but it wouldn't surprise me if it's actually for the person applying to sign.

From the OP's post, it appears that he was given the form(s) to fill out.

If it was actually for a third party, I'd guess that there would be a lot more extra visits until this is widely known.

According to the OP the forms were only given out after the person was already there, often alone.

It looks like imm is just trying to eliminate the idiotic "I didn't know." excuse.

Of course that will still be used as long as you haven't been given a copy of what they had you sign.
