Hi again, more issues I'm afraid.

My wife went to the Chiang Mai Amphur to ask about obtaining the above whereupon she was told there's no such paper?

Now I've read quite a few threads on here and another forum and clearly there is. Perhaps it's a translation issue or maybe the staff just don't know there, in which case it's pretty incompetent.

Would anyone possibly have any details on this or indeed the actual form itself so that my wife can go back to them, as currently she's at a dead end.

She has the custody order from the court for her son, so I'm not too worried if we don't have this but for piece of mind I'd rather have this paper, especially as I've read some ECO's in past cases have specifically requested it. For info, her son has always lived with her at her home address. His father hasn't been on the seen since he was 3 months old.
