I got to the consulate around 9:45 in the am. I took a queue number and there were about 25 people ahead of me in the queue. 20 minutes later I was out the door after submitting documents and paying the fee. The next day I arrived at 2:15 after hearing that sometimes they opened up early. They didn't open up early, but I had my passport and shiny new visa in hand by 2:40. I then had a taxi take me to the KLIA express train that leaves from KL Central. I checked my bag in right their for Malaysia Air and caught the 3:20 train that got me to the airport at 3:50 for a 5:15 flight. The whole trip went well and smooth.

Copy of the passport of the person applying for the visa.

Keep reading beyond No. 3 and you will find this:

7. Copy of Thai identity card (of spouse or parent).

I stayed at http://www.theroyalechulan.com.my/ It was a 10 ringgit (100 baht) taxi ride which was about 2 km to the Thai Embassy. Pavilion shopping mall is a 4-5 minute walk away for shopping, restaurants. food court, grocery shopping etc. Anywhere in the Bukit Bintang, KLCC area is good, especially if your hotel has a view of the petronas towers which the royale chulan had.
