Extension Double Tr Visa

As it is my first time going for extension on my Double entry Tr visa (after being 3 years here anyway )..... i would like to know if immigration give 30 days from the expiring date (7 November ends my first entry ), or from date of demanding for it ?

and do i need 2 forms to fill in or 1 ? passport pictures how many ?

From end of current permitted to stay. The number may very by office used so you should provide that for specific answer - safe bet would be have two if asked.
