VisasPlus Posted February 13, 2012 Share Posted February 13, 2012 It is not possible to download an application form for one of these passports. You need Form SE04. The form is numbered, and issued to an individual. If you require one, then contact the British Embassy or consulate, give your name, and they will obtain one for you. Once the form is completed, make a copy of each page of your passport. Take the form, original passport and copy passport to the Embassy or consulate. They will check the copy against the original passport, and return the original to you. The application is then sent, by the Embassy or consulate, to the UK.
