Kar Sai massage involves the testicles, not the prostate. It is a legitimate, traditional, health related modality. Given the anatomy involved though, discussion of Kar Sai massage often degenerates & gets shut down. I think that's unfortunate but not surprising.

On my first trip the Thailand (back in the actual good old days) had a masseuse do that on me uninvited, at the time thought it was just setting up for a happy ending, however thinking back on it and realising it is for real, it actually was quite interesting. I had a Kra Sai massage at Doi Saket. Quite painful and expensive. Did not notice any benefits, esp no effect on lower back problem which is what I was after. Maybe my prostate is healthier, but no way of knowing.
