Is aware that the price will vary over which operation is required, but hoping that someone with experience can give me one sensible answers and information.

A friend of mine ended up hospitalized in a local hospital up here in Isaan with acute heart attack last night.

They gave him injection thats what saved him. In a few more days they will transfer him two Khon Kaen, where they have heart hospital, where he likely would have to conduct an operation.

The problem is that this man is not insured, so if anyone can estimate what a heart operation will cost? 50,000 or 500,000 thb?

Obviously, cost will depend on the procedure. At least in Khon Kaen, believe it'll be at a regional government hospital so cost will be a lot less (hopefully) than in a private hospital.

For some real ballpark estimates, and at high cost level, do a Google for:

The cost of a triple bypass can range from 150,000 in Chiang Mai Provincial Hospital to over 750,000 at Bumrungrad, no doubt the difference is the same elsewhere in the country based on where you are located and which hospital you use. There is currently a thread in the Chiang Mai forum that describes one posters account of his triple bypass last week and the associated costs, well worth reading. Personally, if it was me, I'd have zero problem with the 150,000 route at the CM Provincial Hospital or similar and I'm an ex Bumrungrad patient.

your goner have to add all the tests to be done

then there's is what surgery he needs,

mobi had a new aota valve fitted he can give you an idea,

i myself had a double by pass and an aota valve fitted in the uk,

that would have cost between £40k-£50k.

then there's the after care plus medication,

so untill they know whats wrong its only a guess.

Assuming this is a government hospital and not private, somewhere in the 100 - 200 L range is likely provided no complications.

With ischemic heart disease there is often an option of open heart surgery in which blood vessels taken from the leg are grafted onto the heart to provide an alternative blood supply and the placement of stents to keep the occluded blood vessels open. The latter is much less invasive and has a much quicker recovery time but due to the high cost of the stent devices ends up costing about the same.

If it's a private hospital in KK then >300K and suggest he go instead to KKU Hospital which in addition to costing less is IMO a far better facility.

Coronary angioplasty is the opening of current arteries (balloon) and stent placement and is indeed very easy procedure (normally done through wrist entry here as first option). You are awake and no pain and the odds are good you will survive (but long term fix is likely less than open heart surgery). But it does require expensive equipment and stents so quite expensive (at Vejthani Hospital in Bangkok - perhaps second rung in price among private hospitals) my price was 300k package and had two non-drug stents (needed another operation which would not have been possible with drug stents that require long period of blood thinning). One overnight in ICU to provide time to remove pressure bandage (reduce pressure every couple hours) and leave in morning.

Having experience second hand my fathers open heart surgery/replacement procedure would take the angioplasty if an option/unknown as it is also a comprehensive exam so suspect it stents do not appear to be useful and open operation needed expect cost would be less than double the price.

KKU is one of the best hospitals in Thailand, private or government. You may not have a 5 star room, but the doctors are the same that work in the private hospitals.

The OP appears to be talking about the Queen Sirikit Heart Center.

It is associated with KKU, but is an independent institution that is located just to the South of Srinakarin (KKU) Hospital.
