Customs Duty on Medical Supplies

I wish to ship from USA to Pattaya, one postoperative walking shoe or boot (not a pair). Not available at Fascino, Pattaya or Bangkok hospitals. This is a medical supply/shoe, any customs duty or special considerations?

Label the item Equipment for the use of a handicapped person only.

It should then come with 0 duty but possibly 7% GST.

Jus don't ship with Fedex, DHL, UPS, TNT or other bastards of this kind.

Use USPS priority but no courier companies that are spies for customs and tax parcel on their behalf (this is the reason why they are allowed to process delivery faster).
