You will extend your 60 day permit to stay your visa gave you for 30 days. Visas are not extended.

You got the form and copies required correct,

a word of warning if you extend the first 2 entries, make sure you use your last entry before the expiry/use before date of the actual visa, which would have been 6 months from date of issue,

How can you say Visa's are not extended when you can 30 days extra on each entry if you pay the 1900 baht and submit the correct paperwork

Look at visa's "enter before" date, that is never extended.

What is extended is your permission to stay. It may seem the same thing to you, but it is not.

a visa is replaced by the permission to stay stamp, the visa is then finished, (used up, deceased, gone, is no more), in the case of a 2 or 3 entry tourist visa each of the entries are replaced by a permission to stay stamp, and then after the last entry is used, or the use by date/expiry date is reached that visa is then finished, (used up, deceased, gone, is no more)

as others have tried to explain you do not extend the visa,( the use by/expiry date always remains the same), you extend one or more of the entries that is allowed on that visa.

A visa gets you across the border only. Your permission to stay or an extension of that permission to stay in the country is given to you by immigrations. The printed expiry date on the visa sticker is never extended. Quite often the visa will expire while you are in Thailand but your current permission to stay IN the country will have more time remaining. Once the visa expires it cannot be used again or extended or renewed. Depending on circumstances, your permission to remain in Thailand might be extended.

As another poster pointed out, you need to make your third border crossing into Thailand before the expiration date printed on the visa sticker. That is set from the time when the embassy or consulate issues the visa and that is when it will expire despite any extensions of stay you may get.

Two separate matters .... visa expiration and permission to stay expiration. Visas for entries into Thailand. Permissions to stay for continued legal presence in the country.
