I shall be going to Jomtien tomorrow for a single entry TM8.

1. Will it be issued the same day?

2. Details needed:passport photo page and extension of stay stamp,and 1 photo.

3 .Half way down the page it asks ' for (purpose of visa)' What are they expecting me to put?

That is a re-entry permit and is issued the same day. If you are e.g. on an extension for retirement, you put that. Photo and copies can be done at the shop beside the office.

You can also get a re-entry permit at the airport 24/7 to save a trip to immigration.
