Affidavit & Photos for Marriage Extension

Just about to do my first one year extension based on marriage at CW Bangkok. Just going through the document requirements on the immigration website, and also section 2.18 of Order 305/2551, there is the following document listed:

"An affidavit must also be submitted confirming the alien’s marital or parental status with a Thai national."

Can anyone tell me what this is, can't find any info.

Also, do the various photos need to be glued onto A4 paper?

They will do the affidavit on their computer by asking your wife the questions needed to fill in the blanks on it. Then you and your wife will both sign a print out of it.

You should glue them on a A4 paper to save the officers time needed do it. I print out my photos on A4 paper.
