I am looking for a vet in pattaya who can help me with dog passport and the paperwork so we can take our dog to sweden.

I've used one that's just south of Tesco on Thepprasit. It's Muang Ake Central Vet Hospital. About 50M before the Texaco gas station on the left as you are headed north on Sukhumvit. Rather large facility and we really like the vet.

Not sure if they can help you, but might be worth a try if you are in the area. They seem quite professional.

Ask the Soi Dogs Trust in Bangkok,I think testing /chipping is there.

As for air transport Air Berlin had good prices but Royal Norwegian are giving excellent prices for Oslo or Copenhagen for the mutts
