EU approves first malaria vaccine

The World Health Organization announced on Friday that it expects to make a policy recommendation this year for the world’s first malaria vaccine, which, if introduced in Cambodia’s, could aid in the eventual elimination of the virus, the Kingdom’s malaria program head said yesterday.

The vaccine Mosquirix, which was developed by drugmaker GlaxoSmithKline, is currently being evaluated by the WHO after getting a green light from Europe’s drug regulator that it’s safe to use in children aged 6 weeks to 17 months in Africa.

“Having a malaria vaccine will certainly help in blocking or interrupting the transmission of the virus between most people but until we hear from the WHO, we can’t introduce it yet,” said Cambodia National Malaria Center (CNM) director Dr Rekol Huy.

While it sounds like a great breakthrough it appears that it's protection is not only limited, but degrades over time. In the African tests it reduced malarial infections in infants by 25%, but the effective protection is noted as being lower when the infants are 18 months old than it was when they were 12 months old, suggesting that the effect wears off (degrades) over time.

Amongst young children the cases of malarial infection dropped by 50%, with no mention of whether or not the effectiveness of the protection degraded over time as well. Note that those numbers are from trials done in 2012 so it is possible that they have refined/improved the formula since then.

From a 2012 report at the The National Center for Biotechnology Information (USA):

CONCLUSIONS:
RTS,S has demonstrated efficacy and safety in Phase 1, 2, and 3 trials, and has the potential to decrease morbidity and mortality from malaria worldwide. Major challenges include determination of the duration of immunity, assessment of its cost-effectiveness, its use in special populations, and its dissemination in endemic regions. Pending further studies, RTS,S has the potential to become the benchmark as the first effective vaccine against malaria.

(My underlining and bolding.)

The main problem with the duration of the protection is that it apparently takes 4 doses (at the current price of $5 US per dose) to be effective. Doesn't sound like much to you and me even if you have to redo it every 2-3 years.

However to the people in the areas where malaria is endemic, that's a lot of money, per family member, especially when it seems that the odds of being infected are still 50/50 at best (in young children) and 75/25 in infants.

This vaccine is however better than anything else currently available, and the research is being sponsored (in part at least) by the Bill & Melinda Gates Foundation so perhaps they will not only be able to increase the effectiveness but reduce the price as well.
