Does anyone have a link where I can download the form for fingerprints and application form. Also how much does it cost for application? I see I need to send a self addressed stamped envelope which is a bit of an issue as I don't have Thai stamps here in NZ. As for the fee can I send a cheque as I guess it is unwise to send cash.
