ubonjoe Posted September 27, 2015 Share Posted September 27, 2015 I assume you have an extension of stay not a multiple entry non-o visa. A TM47 form and your passport is normally all that is needed. Download: http://www.immigration.go.th/nov2004/download/pdf/tm47.pdf But there are a few immigration offices that will want copies of your passport photo page and few other copies of it and your TM6 departure card. The "attachment" is an instruction. Follow the advice provided in post number 4 As said you most likely have an "extension of stay" stamped in the passport. Liquorice Posted September 28, 2015 Share Posted September 28, 2015 If I go in person or mail my 90 day report, they always ask for copies of the pages of my Passport homepage, last extension stamp, last entry stamp, a copy of my TM6 and my last 90 day report receipt.
