On 3: I have never been asked for ALL pp pages, but yes. I saw this written as a requirement elsewhere as well. It probably needs to read: all necessary and relevant pp pages.

All passport pages are not required. The original first entry stamp when first acquiring a non-O visa or entering with one seems to be important. Last time I applied they copied it for me on their own photocopier.

#6 on the list above is unsure if using income method. I didn't bring it last time I applied and it wasn't asked for.

to answer my own post as the OP I presented a letter from a HK Bank addressed to me and as an alternative an invoice to me at my house for my monthly internet charge. I was told both were acceptable
