Changing my daughter's family name in her class records

Hi, I have been living in Thailand for almost 4 years with my daughter. I am a single mom, and my daughter is studying at the same school I am working at. During the start of my first school year at the school, my daughter's homeroom teacher asked me of her passport. Later on, I asked her for my child's name written in Thai so I can have it embroidered on her uniform. After 1 and half year, I realized that the name they have been using of my daughter is my family name (maiden name. and never married. just have a kid), not her father's which is actually what's written on her passport.

Now i would like to ask what I can do to have it changed? Should there be any documents or affidavits I need to submit for them to correct my daughter's name and reflect it on her school records (such as report card etc...) or I can just directly tell them "hey, change my daughter's family name".

Any help, opinion or suggestion, or anyone who had experienced it before, please give me some feedback. Thank you!
