Hi everyone,

I'm sure this has been discussed widely but I cannot see anything in the last 3 forum pages.

A friend advises that the Personal Information Form has now reached Jomtien Immigration and must be completed at each 90 day reporting visit.

Does anyone have a list of the information required and is it acceptable to the Thai authorities to bring a photocopy to subsequent visits assuming the information supplied hasn't changed?

I always assumed it's the same for everything. the information isn't at all important... it's the process of filling in the form and handing it in. the 90 day letter is the same. just like in "school".

Yes this form is asked in Jomtien too now,

but not at each 90 day reporting, only once.

Just another hoop and of course, it appears.. some places follow it and some do not...

Although the form is in use at Jomtien since June 22, it seems there is no real requirement to fill it out. From all reports I have received from my fellow expats here in Pattaya is that it is handed out at the info desk where you get your queue number sometimes - some report not getting it and some getting it on the same day, but were there at different times. I went in to do my annual renewal of extension and was not handed the form or asked for it at the "retirement" desk.

The next day after picking up the passport, I went to info desk to get queue number for 90 day report - the IO started to hand me the form and said to me "bring it back next time" - but after informing her I had the form completed already, she said hand it in at the 90 day desk. After completing my 90 day report without being asked for the form, I offered it to the IO and he took it placing it on a stack of others - he didn't look at it.

Since then I have had several expats tell me either they were not given the form to complete or they were given the form, but no one at the desks asked for them. Those that did turn in the form, said they were not asked for it, but it was taken when offered - like my experience, no one looked at it - just placed it on a pile of others that had been turned in. Two folks that went in last week to do 90 day told me they were not given the form and no IO asked for it.

I had printed out the form before doing my 90 day report a few weeks ago at Jomtien. Handed it in along with my passport. The officer put it in a stack on his desk without looking at it. Some of the people in the queue before me had it, some did not. It didn't seem to matter either way. No one was asked for it.

They ask for some specific information, such as a Thai friend's name & telephone number or bank account numbers (for certain visa holders ??), some of which most people seem to leave blank anyway. So it goes beyond a simple survey.

The last time I remember them doing a general census (more than 10 years ago ??) someone came to door to door, but didn't ask those sort of questions.

You might be right. The forms I saw had been photocopied on the back of other people's details, so they didn't look very official. It also seems very random - at CW last week, some people were being given the form while others weren't. I still think it may just be someone's bright idea about gathering information for statistical analysis.

Someone needs to post a blank info form on the Forum. I would but I am in the USA for two or three more weeks.

The problem is there is several versions of the form. One form does not work at all immigration offices.
