Bank book with 800K seasoned for 3 months.

Bank letter confirming the above no more than 3 days old

The double sided form which I forgot the name of filled in with a pinned recent 4x6 cm photo on it with light blue background.

1 extra photo

Passport

Signed copies of the last 3 months bank book transactions pages

Signed copies of Passport photo page along with previous multiple entry stamp page

Proof of address or Rental agreement contract (which I was asked to produce last year)

Anything that I missed?

If in Chiang Mai, you may need proof that your landlord has filed a TM30 form for you, simply having a rental contract may not be enough.

Yeah... I've heard that some offices may require that but I don't think Udon is one of them or at least wasn't last year. Cheers
