Yes, it's the first page of an actual application rather than just looking at the information pages, which as far as I can see do not mention the multiple entry option.

As mentioned on the US Embassy website, you may need a certified copy of your passport, depending upon the wishes of the Amphur in question. As for the choice of Amphur it can be anywhere, doesn't have to be where you live or come from. However, best check in advance that they are willing to marry aliens as in these times of COVID fear things are not always what they were.

As with most things in Thailand the easiness is a variable dependent upon the location of the Amphur, the person you are speaking to, the phase of the moon, etc. However, it is not beyond most people to accomplish it. Best place to start is the website of the Embassy of your country, you will certainly have to interact with them to some degree so may as well start there. Time to go home and get a new one, on the bright side it's springtime and the weather should be fine. The embassy could give you an ETD, Emergency Travel Document, but that is only good for travel after your passport expires, not for hanging around in Thailand waiting for a new one to arrive. Presumably you also have permission to stay issues as well?

Did she actually try applying for a replacement/copy? As an example the first page of the WA application asks for your personal details including telephone Country Code and Country address. No suggestion of applications only welcome from Australians in Australia. -

Gov.UK says allow up to 11 weeks, likely it would be quicker than that but probably best not to plan any travels for that time. -

Indeed correct. It is worth noting however that when they say "in person" what they actually mean is "by a person", i.e. not by mail. So the person can be anybody the applicant chooses and provides with the relevant authorisation; wife, friend, taxi driver or indeed the ever popular (on this forum anyway) "agent". -

I had suspected upper GI bleeding. The stomach inspection found erythematous gastritis (inflammation) and an ulcer. 6 weeks on Omeprazole and the ulcer has gone and the gastritis is improved.

The procedure is nothing to be scared about, I had one last weekend and even without sedative, just a throat spray, it's perfectly bearable, with sedative it's hardly noticeable at all as my first was but this time I had to drive home afterwards.
