A couple of minutes on the "Ministero della Salute" website and it appears Thailand is on "List E" which is all countries not mentioned on other lists and it's stated that "From these countries it is not possible to enter Italy for tourism reasons". There is talk of a "travel corridor" between Italy and Phuket but that seems to be intended for tourism in the other direction, if it would work to rather than from Italy remains to be seen.

If you look at the Thailand Pass website and follow the Test & Go hotel links there are hotels all over the country. -

It's mainly Thai expats that are 2nd class. In many/most countries you do an online application and send your passport. The system in Thailand has been devised to comply with the need for you to have access to your passport at all times. I suppose the reason VFS is involved is primarily to cancel your old passport when you get your new one, as they don't trust you with a pair of scissors to do it yourself.

Colour. From GOV.UK; You must also bring your current passport with you when you apply, and a full colour photocopy of the entire passport (every page including blank pages).

If they actually refused your/his application then wait 4-6 weeks and then apply again for an ETD. Get a new passport for him whilst in the UK.

He can drive on a full Thai license for 12 months. However, his first Thai license will be "temporary" so that might raise some issues. See here also; https://www.gov.uk/driving-nongb-licence
