Changing stamps to new passport.

Copies of both passport photo pages. Copies of at least your most recent extension of stay stamp and most recent arrival stamp in the old passport. Some offices will want a copy ever page that has a visa or a stamp for Thailand on it. -
You would not have apply for a new extension after getting a new passport unless they shortened your extension due to your passport expiring. Immigration will transfer your stamps to the new passport. In my opinion you should have your stamps transferred shortly after getting the new passport. Immigration does not have a fixed rule for when it should be done.

A TM30 report is not required now since you are entering with a re-entry permit and will be returning to the same address. The rules were changed in June of 2020.

No Your 90 day report would be due 90 days from the day you enter the country. The online reporting site you posted a link to is gone now. New site is here. https://tm47.immigration.go.th/tm47/#/login You have to register to get on the site. Click apply near the bottom of the page.

If you have been extending a non-o visa entry based upon retirement no health insurance is required when entering the country or for your extension application.

O-A Visa from home country

After applying for and getting a Non-OA visa you do not need proof of insurance or anything else for a year other than proof of insurance when entering the country. A OA visa is a multiple entry visa that allows unlimited one year entries for one year from the day it is issued. It is possible to get a total stay of almost 2 years from it by entering the country just before the visa expires, -
Children under the age of 15 are not fined for a overstay and up the 18 of are not banned from entering the country for an overstay more than 90 days. There is still a requirement for 90 day reporting if the they are legally staying in the country. Under the age of 15 they would not fined for not doing them.

Immigration will only accept 12 months of proof that you transferred your income into a Thai bank from abroad. -

The Phuket immigration volunteers website states the combination option is available there. See: https://piv-phuket.com/long-stay-extensions/retirement/ You will need to proof the the 12 months of income transfers came from abroad. -

As far as I know re-entry permits are not available at any of those border crossings. The Aranyaprathet/Poi Pet was the only crossing with a re-entry permit desk. Probably best to get one at a immigration office to be sure you have one.l -

That is currently unknown. I have seen nothing stating it. Only suggestions to get a booster. -

Ban Laem and Ban Pakard are the same border crossing. Ban Pakard is the district where Ban Laem is located. -

Some embassies and consulates want a ticket out of the country within 60 days to apply for a single entry tourist visa. If needed you could use a one way ticket to anywhere to meet the requirement.- 1 reply

You can apply for a 30 day extension of a 30 day visa exempt entry at immigration.

The medical certificate is only required for a work permit application. No all work permit offices want a syphilis test. The medial certificate is not a requirement to apply for a extension of stay at immigration.

It is also known as the Ban Laem crossing and it is open. A visa run company has been doing the runs 3 times a week from Bangkok to there for some time now. -

If a person applies for the 60 day covid 90 extension up to the 25th of this month the 60 days would end in September.
