Their are new forms for a work permit application forms or renewal. They are still fillable. At one time the DOE had the work permit forms for download but not any more. You should contact the local DOE office to get the form.

A TM30 report of address report would only be needed if entering with a new visa. If he uses re-entry for a existing extension of stay or a multiple entry visa it would not be needed. -
You can put about anything in valid reason space for multiple entry tourist visa. For example it could be visiting friend or family and etc. You can only apply for one 30 day extension per entry from f a tourist visa or visa exempt entry. Normally the e visa application is approved within about a week of the day your application is accepted. You could enter visa exempt and get a 45 day stay on or after October 1st that can be extended for 30 days. Then do a border hop to get another 45 day visa exempt entry and extend it for 30 days. Then another border hop to get another 45 day entry.

It is same extension but for a visa exempt entry it would be done at Laksi IT Square office. A re-entry permit would not allow a 2nd 60 day entry. A re-entry permit is only issued to the day the current permit to stay it is issued for ends.

At this time nothing no time frame for being fully vaccinated. I assume if you got a booster shot that would reset the start date of your vaccinations. -

The 90 days start again when the 90 non immigrant visa entry stamp is done after the application is approved. The issuance of the visa resets the original entry to the country. -

That is not required for the 60 day extension to visit your Thai wife or children. It can be applied for with any category of visa entry or a existing extension of stay. It is set by the day your current extension ends not the day you apply for a new one. You will have to have proof of the new insurance requirements, -

A covid 19 test is has not been required since July 1st if fully vaccinated. There is no limit on how old the vaccination can be at this time. -
The problem could be not having a bank book showing you had the 800k baht in a Thai bank for 2 months before and 3 months after the application and then 400k baht baht for the rest of the year. If you have that it should be a problem to apply at CM immigration if you submit a TM30 form with CM address on it.

Some people have applied for a e visa at a embassy or official consulate in their home country while here. But there are some warnings about it on the e visa site and embassy/consulate websites. Personally I see no big advantage of doing the e visa while here rather than at a nearby embassy or consulate. You would still have to leave and re-enter the country and the e visa application has requires flights in and out of Thailand that is not required at a nearby embassy or consulate,

Not a problem to leave on one passport and re-enter on a different passport by air. It would not be possible at a land border crossing due to the country you are entering will want to see a departure stamp for Thailand to enter and then the same when returning to Thailand. Phuket immigration was wrong about getting a 45 day extension of your visa exempt entry. It is possible to apply for a non immigrant visa at immigration with a visa exempt entry (you do not qualify for a 15 day visa on arrival). Whether you could apply for a non-ed visa at immigration would depend upon the type of school you are attending.
