As something of a partial answer to my own question, my friends took a two hour class from the Nipa restaurant at the Landmark Hotel: http://www.landmarkbangkok.com/dining_nipa.html The class was 450 baht/person and mostly consisted of a non-English speaking Thai showing them each step individually as they were ready for it. They enjoyed it, but were fairly exhausted after 2 hours of concentrating on the detailed carving, so I think one would need to be pretty dedicated to commit to the longer (and more expensive) classes. There is also an old lady (in her 80s something) who used to work in the royal palace and now teaches fruit carving in her house in Bang Lamphu. She don't speak a word of english though, so nothing for tourists. There is also a book published about her, but I can't remember her name at the moment.
